LIABILITY WAIVER

Assumption of Risk:

I acknowledge and understand that participation in physical activities organized by Coach P Inc carries inherent risks. I recognize that these risks include but are not limited to physical injury, illness, or property damage. I understand that it is my responsibility to ensure that the participant is in good health and physically capable of participating in these activities.

Release of Liability:

In consideration of my child being allowed to participate in Coach P Inc physical activities, I hereby release and discharge Coach P Inc, its agents, employees, and representatives from any and all claims, liabilities, losses, damages, or expenses that may arise as a result of participation, except for those caused by the gross negligence or willful misconduct of Coach P Inc.

Medical Treatment Consent:

In the event of an emergency, I authorize Coach P Inc and its representatives to seek and secure medical treatment for the participant as deemed necessary. I understand that Coach P Inc will make reasonable efforts to contact me in such situations, but if I cannot be reached, I consent to the medical treatment deemed necessary by licensed medical professionals.

PHOTO/VIDEO WAIVER

Coach P Inc Homeschool P.E. and its authorized representatives to take photographs and/or videos of my child during the program's activities. I understand that these photographs and/or videos may be used for

promotional purposes, including but not limited to website content, social media posts, brochures, and other marketing materials.

I waive any right to inspect or approve the finished photographs and/or videos or the use to which they may be applied. I also release Coach P Inc Homeschool P.E., its employees, agents, and representatives from any claims, demands, or causes of action arising out of or in connection with the use of these photographs and/or videos, including but not limited to any claims for invasion of privacy or defamation.

I understand that my child's participation in the program is voluntary and that my decision to sign this waiver will not affect their participation. Furthermore, I acknowledge that my child will not receive any compensation for the use of these photographs and/or videos.
