FIELD DAY

Parent Supervision Required:

This is not a drop-off event. Parents/Guardians are responsible for supervising their children at all times outside of designated supervised field activities.

LIABILITY WAIVER

Assumption of Risk:

I acknowledge and understand that participation in physical activities organized by Coach P Inc carries inherent risks. I recognize that these risks include but are not limited to physical injury, illness, or property damage. I understand that it is my responsibility to ensure that the participant is in good health and physically capable of participating in these activities.

Release of Liability:

In consideration of my child being allowed to participate in Coach P Inc physical activities, I hereby release and discharge Coach P Inc, its agents, employees, and representatives from any and all claims, liabilities, losses, damages, or expenses that may arise as a result of participation, except for those caused by the gross negligence or willful misconduct of Coach P Inc.

Medical Treatment Consent:

In the event of an emergency, I authorize Coach P Inc and its representatives to seek and secure medical treatment for the participant as deemed necessary. I understand that Coach P Inc will make reasonable efforts to contact me in such situations, but if I cannot be reached, I consent to the medical treatment deemed necessary by licensed medical professionals.

PHOTO/VIDEO WAIVER

I grant Coach P Inc. permission to capture and use images or videos of my child during Field Day for promotional purposes, including social media and website content. I release Coach P Inc. from any claims arising from the use of this media and acknowledge that no compensation will be paid. I understand that my child's participation in Field Day is voluntary and not contingent upon this waiver
